The addiction to over the counter decongestant nasal sprays is known as Rhinitis Medicamentosa or RM. As the name implies, RM refers to rhinitis (congestion) caused by medication (Medicamentosa). Prolonged use of over-the-counter (OTC) decongestant nasal sprays which contain topical vasoconstrictors such as Oxymetazolinehcl, Phenylephrine hcl and Xylometazolinehcl are known to cause Rhinitis Medicamentosa.

When topical decongestants are used for more than four to five consecutive days, rebound congestion quickly develops, thereby leading to the prolonged use of the sprays. Rebound congestion is the result of abnormal enlargement and swelling (hypertrophy) of the nasal mucosa, which effectively blocks the nasal airway and creates extreme discomfort.  The congestion is momentarily relieved by the administration of another dose of the decongestant nasal spray. As soon as the temporary effect of the last dose of spray wears off, the swollen nasal mucosa again blocks the airway and another dose of spray is required to provide relief. The commencement of this cycle represents the initiation of the addiction (Rhinitis Medicamentosa).

The Services Manager continued, “Only those who have been addicted to these nasal sprays can understand the impact it has on their lives. At Rhinostat, we have recorded many cases of individuals who have suffered from decongestant nasal sprays for more than four decades. Since the nasal spray itself is the root cause of the problem, the only effective way to eliminate rebound congestion it is to stop the use of the sprays. Due to the extreme discomfort associated with the abrupt withdrawal, very few individuals can bear it. There are no FDA-approved therapies nor drugs particularly approved for RM treatment. Typically, these patients are given a course of intranasal and/or systemic steroids and are told to discontinue their use of the decongestants. In some cases, surgery to reduce the turbinates is done. Notwithstanding the treatment prescribed, the cornerstone of the therapy is always the same. Patients must discontinue their use of the sprays. It is this aspect of the treatment that presents the problem for these patients. Drs. Graf, Enerdal, Hallen and Juto studied the use of the steroid nasal sprays in treating rhinitis medicamentosa in 1997. Their phenomenal study published in the Journal of Clinical and Experimental Allergy forms the basis of the use of steroid nasal sprays in treating rhinitis medicamentosa.
