Trinity Healthcare is a Fort Worth, Texas based company specializing in the operation and management of senior housing in the Texas market.

The facility name will be changed to Autumn Winds Living & Rehabilitation. While the name will be changing, residents will continue to see all the familiar faces providing personalized care.
The goal is to make the transition as seamless as possible for residents and their families.
